- From Duchess’s Stash, which you can find as part of a Miscellaneous Quest when you arrive in Welch in the Ash Heap region (9.09% chance to obtain it from the Stash)
- Farming Daily and Repeatable Quests in The Forest region

Buying it from Modus Medical Terminal
The best and guaranteed method to obtain the Psycho Recipe in Fallout 76 is by purchasing it from the Modus Medical Terminal inside the Whitespring Bunker.
To reach the Vendor, teleport to The Whitespring Service Entrance as it is the quickest way to him.
Once you spawn, head inside the Service Room and go past the security check.
Follow the signs leading to the Medical Bay area.
There, you will find Modus Medical Terminal. Use the Trade option to get the Psycho Recipe.
What if you are a new player?
If you are a new player, you can either use the above-listed methods to obtain the Psycho Recipe or ask some other high-level player to get and drop it for you.
